Preparing for Surgery

A Health History Nurse will call you prior to your surgery to collect information about your health to expedite your admission process.

To expedite this process, please complete your One Medical Passport (www.OneMedicalPassport.com).

To complete your One Medical Passport: 

1. Select “Register”
2. After registering, select “Advocate Surgery Center” or enter facility ID number: 1207.
3. You may then begin filling in your medical history
